What this site is
This is an early-access sign-up page for FolkMatter, a product in development. It collects an email address and a couple of survey answers so we can follow up when testing opens. It is not the FolkMatter application, and it does not accept medical records, health documents, or any other health information.

Nothing on this site is medical advice. FolkMatter does not diagnose conditions, decide medication changes, or replace the judgment of a doctor or pharmacist. Any example shown on this page uses fictional information and does not describe a real person's care.
Signing up
Joining early access does not guarantee access to a future product, a specific release date, or any particular feature. We may contact you by email about testing or feedback opportunities, and you can ask to be removed at any time by emailing hello@folkmatter.com.
Changes
These terms may change as FolkMatter develops. We'll update this page when they do.
